Dr. Balyeat is a board certified ophthalmologist with additional fellowship training in the
management of disease affecting the macula, retina and vitreous.  He specializes in the
surgical management of retinal detachment, macular holes, epimacular membranes ("macular
puckers"), the medical and surgical managment of diabetic retinopathy and inflammatory eye
disease (uveitis) and the medical management of age-related macular degeneration.  He is a
fellow of the American Academy of Ophthalmology and a fellow of the American Society of
Retinal Specialists.  Dr. Balyeat has been in practice in Tulsa with the ophthalmologists of The
Eye Institute since 1988.
Dr. Balyeat's practice is open to patients of all ages.  We are often asked to perform second
opinions especially prior to cataract surgery.  Although most of Dr. Balyeat's patients are
referred by other ophthalmologists and primary care physicians, his medical practice is open
to self-referred patients with known retinal conditions.

St. John Medical Center is located in mid-town Tulsa at the intersection of 21st Street and Utica Avenue.

From the Airport:
To get to St. John from the airport, take Airport Drive to the light at Virgin Street.  Turn right, go
approximately 100 yards, turn left at entrance to Oklahoma 11 (Gilcrease Expressway).  Take Oklahoma
11 south to Interstate 244.  Turn right (west) onto I-244. Exit at Utica Avenue.  Drive south to 21st Street;
turn left.  Drive east to Wheeling Avenue; turn left. Both entrances to the visitor parking arcade serving the
Williams Medical Plaza and the hospital are located just west of Wheeling Avenue.

From the North:
To get to St. John from north of town, take US 75 to I-244 east. Exit Utica Avenue.  Drive south to 21st
Street; turn left. Drive east to Wheeling Avenue; turn left.  Both entrances to the visitor parking arcade
serving the Williams Medical Plaza and the hospital are located just west of Wheeling Avenue.

From the South:
To get to St. John from south of town, take US 75 to US 64/51 east.  Exit Utica Avenue.  Drive south to
21st Street; turn left.  Drive east to Wheeling Avenue; turn left.  Both entrances to the visitor parking
arcade serving the Williams Medical Plaza and the hospital are located just west of Wheeling Avenue.

From the West:
To get to St. John from the west, take I-44 east to I-244 east/US 75 north.  Follow US 75 north to US
64/51 east. Exit Utica Avenue.  Drive south to 21st Street; turn left.  Drive east to Wheeling Avenue; turn
left.  Both entrances to the visitor parking arcade serving the Williams Medical Plaza and the hospital are
located just west of Wheeling Avenue.

From the East:
To get to St. John from the east, take US 64/51 or I-44 west to Utica Avenue.  Drive south to 21st Street;
turn left.  Drive east to Wheeling Avenue; turn left.  Both entrances to the visitor parking arcade serving
the Williams Medical Plaza and the hospital are located just west of Wheeling Avenue.
